Olympus manufactured reusable endoscopes and colonoscopes that required proper cleaning between patient uses through a reprocessing procedure. When this cleaning was incomplete, bacteria could remain on the devices and infect subsequent patients. Health Canada issued recalls in March 2026, and Olympus issued urgent safety notices in October 2025 regarding contamination risks. Patients exposed to these improperly cleaned devices developed severe infections, sepsis, and other life-threatening complications.
Who's Eligible?

Patients who underwent colonoscopy or endoscopic procedures within the past 10 years using Olympus equipment and developed severe infections or sepsis within one month of the procedure
